A listing of forms required and their due dates can be found on this Cover Sheet.
The following documents are to be submitted EVERY QUARTER via email.

  • FY 15 CAPSAW Partner Agency Quarterly Report Form 
  • CSBG Demographic Report
  • FY 15 CSBG Unduplicated Client by Locality Report   

The following documents are to be submitted for the 4th Quarter by July 15, 2015 via email.


An original signed copy of the following document must be submitted by the due dates listed below EVERY QUARTER to:

Submission Deadlines
  • First quarter submission deadline is October 15, 2014 for the period of July 1, 2014 through September 30, 2014
  • Second quarter submission deadline is January 15, 2015 for the period of October 1, 2014 through December 31, 2014
  • Third quarter submission deadline is April 15, 2015 for the period of January 1, 2015 through March 31, 2015
  • 4th Quarter Submission are due on July 15, 2015 for the period of April 1, 2015 through June 30, 2015
